From benefit to bill

Coverage follows the drug and the reason for using it

The same medicine can receive a different answer for a different diagnosis. Use the exact generic name and prescribed reason in every plan call.

Read the approval evidence

The Medicare distinction

As of September 2026, Medicare drug-benefit law still excludes medicine when it is used only for weight loss.

Coverage may still apply when that drug is prescribed for a different approved use, but each plan’s drug list and approval process still control the actual claim.

Coverable does not mean automatically covered.

Questions a plan can answer

  1. Is this generic name covered for this diagnosis?

Spell both out and ask for the answer in writing when the plan offers that option.

  2. Is prior approval required?

Ask what records the plan needs and who submits them.

  3. Which form and pharmacy count?

Coverage can depend on the product form and the pharmacy network.

  4. What is the member cost?

Ask whether you will pay a flat copay, a percentage of the price (called coinsurance), the full price until your deductible is met, or a cash price outside the plan.

If the answer is no

Ask whether the plan has an exception or appeal route and what record starts it.

Keep cash prices separate from covered prices, and ask the plan whether money you pay in cash counts toward your deductible or yearly total.
